Optus blames software upgrade for outage

Optus says it still does not know why hundreds of Triple Zero calls failed to go through during last week's nationwide outage. The company's embattled CEO today told parliament it had no emergency planning for such an event, as demands for compensation from angry customers approach half a million dollars.
